The alt text appears in a blank box when an image is unable to load for some reason, and determines what a screen reader will read off for non-sighted or low vision visitors to the site.
The decision whether or not to add alt text is dependent on both the meaning of the photo itself, as well as how it is being used on the page. This decision tree can help you decide when and how to write good alt text.
